Job DescriptionMinimum Qualifications
Permanent Qualifications: Candidates must possess or be eligible to
obtain a license to practice as a Licensed Practical Nurse in New
York State.
Transfer Qualifications: Candidates must currently be a permanent
Licensed Practical Nurse.
All candidates must also possess a valid Driver's license. Upon
appointment you must submit proof of a valid license to operate a
motor vehicle in New York State. If you fail to maintain your
license to operate a motor vehicle in New York State, appropriate
measures may be pursued to remove you from this position.
Duties Description
This LPN is needed to provide hands on nursing coverage for the
medically involved individuals who reside at Valatie South IRA.
Nursing / Clinical oversite will be provided by the Register Nurse
1 with administrative oversite provided by the Registered Nurse
Supervisor 1. This LPN will need to report to the base location of
Cove Road IRA a minimum of 2x/month. There is no mileage
reimbursement from the base to the other work site(s).
Duties include, but are not limited to:
- Assist with nursing tasks within their scope of practice.
- Ensure that all medications and treatments required on that
shift are performed and implemented as written, and the data is
recorded.
- Provides documentation as needed.
- Ensures that the following documentation is accurate and
available such as The Ready to Go, monthly MAR, Consents, Current
Scripts
- Maintains tickler for OTC expiration dates and OTC orders for
the individuals.
- Monitors/supervises individuals with insulin administration,
and with PO daily medications/treatments.
- Provided first aide to individuals and monitor vital signs as
needed and reporting to the Register Nurse12 their findings.
- Acts a liaison between the IRA Residential staff and the
Registered Nurse1.
- Purchasing and organizing med room supplies
- Other duties as assigned.
Additional Comments
This title is part of the New York Hiring for Emergency Limited
Placement Statewide Program (NY HELPS). HELPS Program titles may be
filled via a non-competitive appointment. This means that you do
not need to take an exam to qualify, but you do need to meet the
minimum qualifications of the title.
At a future date (within one year of permanent appointment),
employees hired under NY HELPS are expected to have their permanent
non-competitive employment status converted to permanent
competitive status. You will not have to take an exam to gain
permanent competitive status.
- Please note, the posted Hiring Salary includes the sum of the
statutory hiring rate for ISU Grade 10 ($43,768), along with any
special salary treatments in effect for this position in this
location, such as a Geographic Differential ($13,000), increased
Hiring Rate ($5,680), and Mid-Hudson Adjustment ($1,650). The
posted maximum salary ($68,358) Job Rate includes the previously
mentioned Geographic Differential and Mid-Hudson Adjustment.
- If you are currently a New York State employee serving in a
permanent competitive qualifying title and eligible for a transfer
via Section 70.1 of the Civil Service Law, you may be appointed to
this title as a permanent competitive employee.
- For CSEA positions: Local Bidding Agreements will be
honored.
- Preference will be given to OPWDD employees impacted by
